Find Your Starting Point

What’s influencing your health?

Many health conditions don’t develop overnight.

The foods we eat, how we move, sleep, handle stress, connect with others, and interact with our environment can influence our health over time.

This quick quiz isn’t a diagnosis. It’s a chance to reflect on the patterns that may be supporting your health and discover where small changes could make a meaningful difference.

Takes about 3 minutes.

Step 1 of 3

Your Why

A personal reason for taking care of your health can help make change feel meaningful and guide where you focus. You don't need to have it all figured out. Just share whatever matters most to you right now.

Some examples are:

  • I want to wake up in the morning feeling rested.
  • I want more energy for my family.
  • I want to prevent future health problems.
  • I want to feel better day-to-day.
  • I want to manage an existing condition.
  • I want to stay healthy as I age.
  • I want to be able to do things my parents can no longer do.

Step 2 of 3

Your Health Habits: The Six Pillars of Lifestyle Medicine

The six pillars are everyday habits that can influence your health. For each question, move the slider from 1–10 based on where your current habits fall between the two statements.

  • I spend much of my day sitting or inactive and rarely exercise or move regularly.

I am physically active regularly and include a mix of aerobic, strength, flexibility, and balance activities.

  • Most of what I eat is highly processed, and I eat few fruits, vegetables, whole grains, beans, nuts, or seeds.

Most of what I eat is made up of a variety of whole or minimally processed plant foods, such as fruits, vegetables, whole grains, beans, nuts, and seeds.

  • I regularly get too little sleep, have an inconsistent sleep schedule, or wake up feeling tired and unrested.

I consistently get enough quality sleep to wake up feeling rested and restored.

  • I often feel overwhelmed by stress and have few ways to manage or recover from it.

I recognize when I am stressed and regularly use healthy strategies that help me manage stress and recover.

  • I often feel isolated or disconnected and have few meaningful or supportive connections in my life.

I regularly connect with people, community, pets, nature, or spirituality in ways that support my sense of belonging and well-being.

  • I regularly use tobacco, drink more alcohol than is recommended, or use other substances in ways that may harm my health.

I avoid tobacco and other harmful substances and limit or avoid alcohol.
